Use this form to request the creation of new communities and/or collections in cIRcle and/or to request authorization for specific users (cIRcle logins) to upload files to the specified communities and collections. Please fill out the following information and click the ‘Submit’ button. An asterisk next to a field name indicates that the field is required. Workflow authorization requests are processed in the order received. You will be emailed when your request has been completed.
